Globally, a patient loses a limb due to diabetic foot ulcers every 20 seconds. These ulcers are considered one of the most difficult wounds to treat, with serious consequences and complications to patients. The problem typically begins with a loss of feeling in the foot due to diabetic nerve damage, which can prevent individuals from feeling pain. So similarly to how you could wear a hole in a sock or a shoe, diabetics with nerve damage could wear a hole in their foot without noticing. Additionally, in some cases, diabetes can cause blood vessels to narrow which reduces blood flow to the extremities. This reduced blood flow makes it difficult to heal wounds, which is why even a small cut or scrap on the foot could lead to a diabetic foot ulcer. The reduced blood flow also decreases the effectiveness of fighting infections. Diabetic foot ulcers often take weeks to months to heal, with one-third of ulcers never healing and 50% becoming infected. For 20% of patients, stagnant healing, necrosis, and/or chronic infection will lead to limb amputation due to risk of life-threatening complications. The big picture is that these ulcers are just a portion of chronic wounds, which are often overwhelmed by infection, have stagnant healing and have restricted healing resources at the wound site.
Wound healing is extremely complex with multi-step cascading reactions. Many wound products are bio-chemistry based and must be used at appropriate times to avoid harming other healing steps. Patients often require multiple products for infections or healing and figuring out the best application timing is critical. The sad truth is that even with all these products people with chronic wounds are still dying.
